Seniors Coaching Seniors is a UHF grant funded program that helps connect volunteers and recently discharged patients aged 65 years and over. \u00a0These volunteer coaches provide telephone support and guide their peers through their doctor\u2019s recommendations.\u00a0 All coaches are trained by the Department of Care Transitions Clinical Team, including a registered nurse and licensed social worker.<\/p>\n<p>We encourage all staff to spread the word to eligible patients. Let them know a Volunteer Coach will be reaching out to them after discharge to assist them.
